Please print out and complete our new patient paperwork found below. We have changed our medical history form and review system. We ask that you arrive 15 minutes prior to your appointment with Dr. Grace with this paperwork completed so we have adequate time to enter the information into our system. Don't hesitate to call us with any questions.

RECOVERY
WHAT IS IT LIKE?

If discomfort occurs it can last for a few days to a week. Walking, low impact exercises, and stretching are encouraged starting the day of treatment and help people get better faster. Strenuous activities should be avoided for two weeks, but after that there are no restrictions.
